After the preventive care visit, Personal Wellness will continue to support our patients by proactively coordinating and helping manage their follow-up care needs.

• Ensure patients get the follow-up care as outlined by the Preventive Care and Wellness visit
• Assist in appointment scheduling
• Help with transportation needs
• Act as a “care navigator”
• Coordinate care between specialists
